Incident: 60 cm Panel Falls from KLM 777 onto Car on Departure from Osaka, Japan
A panel weighing around 4 kg fell off a plane shortly after takeoff Saturday at Kansai International Airport and hit a car on a road in Osaka, the transport ministry said. No one was injured by the object. The ministry will send safety inspectors to look into the cause of the mishap, officials said. Japanese maintenance crew must have used chop sticks to fasten panel. It was lunch time.

Can we blame that on the mechanics? yes we can, once you secure every panel the way it is suppose to be, it will never ever will come apart, i am an A&P since 1968 and that never happened to me, you just has to be very careful working on any aircraft.
Thankfully this panel didn't fall off over North Korea, or little rocket man would've used the opportunity to blow another ICBM at some innocent fish.
