Failure modes and effects analysis is an essential part of aerospace engineering and design analysis. That Boeing short circuited this means they ought not be allowed to build aircraft at all. The senior executives of Boeing need to be all fired and given nothing, no bonuses, no golden parachutes, tell them they are luck to have escaped jail time.

The CEO, the COO, the CFO and the entire board of directors ought to be held liable. They had to be aware of the issues and ordered and/or approved the shortcuts and "fixes" that created this dangerous aircraft. Don't hold your breath however for them to be held responsible.

Imagine you are in a self driving car on autopilot, admiring the view, and the cars sensors and software determine that a rapid swerve to the left is needed and the car immediately and without warning does this. You crash into oncoming traffic. Imagine there is a button or switch that can disengage the swerve function. According to Boeing, this accident is the drivers fault because you did not active the switch rapidly enough to avoid the collision. Utter crap and rubbish. Boeing is at fault, it is NOT as they claim, a training issue. The aircraft and its design was totally at fault-period.

The Boeing 737 max aircraft ought to be grounded until the problem is fixed. This is not a human operator training issue, it is faulty avionics and controls that should not respond to a sensor failure with a dive into the ground. Training my A$$.
