It's a disaster and another black mark on General Aviation. I'm not pointing fingers here but all too often, I have witnesed good to great pilots spending too much attention inside the cockpit. In heavily conjested areas or areas of questionalble activity, "we should be looking outside the window and flying the plane". One of my pre-briefings to passengers is - "I am depending upon you to look outside the window, look for birds and other planes". It helps and my passengers like finding something before I do.
